目的:探讨Dynesys动态稳定系统(弹性固定)治疗腰椎间盘突出症的临床应用及治疗效果。 方法:分析20例应用Dynesys动态稳定系统治疗腰椎间盘突出症的临床结果和影像学资料。日本整形外科学会(JOA)评分评估临床疗效,腰椎过伸和过屈、侧位X线片评估腰椎活动度和腰椎前凸角(Cobbs法)。手术采用脊柱导航系统及透视下固定动态稳定系统。术后及随访复查X线及MRI检查结果和JOA评分。观察Dynesys动态稳定系统对固定节段活动度的保留情况及邻近节段的退变情况。MRI检查观察邻近节段退变情况。 结果:术后随访 6~29个月,平均19个月。20例患者术后疼痛缓解情况良好。JOA评分均显著改善。随访时复查影像学检查未发现明显的邻近节段的退变,保留了相应节段的活动度。 结论:使用Dynesys动态稳定系统的患者术后可早期下地活动。手术治疗后约2年的随访中发现患者的日常生活明显的改善,生活质量得以提高。得出结论Dynesys动态稳定系统是治疗腰椎间盘退变性疾病的有效方法,可以获得满意的临床疗效和可有效的保留固定节段一定的腰椎活动度,有效防止相邻节段的退变。 |

Objective: Explore the Dynesys dynamic stabilization system (elastic fixed) treatment of prolapse of lumbar intervertebral disc protrusion: clinical application and therapeutic effect. Methods: Dynesys dynamic stabilization system in treatment of analysis of 20 cases of prolapse of lumbar intervertebral disc protrusion: clinical results and imaging information. Japan Society of plastic surgery (JOA) scoring assessment of clinical efficacy, lumbar extension and flexion, lateral x ray evaluation activities of lumbar spine and lumbar lordosis angles (Cobbs). Surgical navigation system and dynamic stability of a fixed system under the perspective of the spine. Postoperative and follow-up review of x-rays and MRI and JOA scoring the examination results. Observe the Dynesys dynamic stabilization system for fixed segments of activities reservations and for adjacent segment degeneration. Observation of MRI to check the adjacent segment degeneration. Results: 6-29 months of follow-up, an average of 19 months. 20 cases of patients with postoperative pain relieve in good condition. JOA scoring improved dramatically. Follow-up review of imaging examination found no noticeable adjacent segment degeneration, retains the corresponding segments of activity. Conclusion: Using the Dynesys dynamic stabilization system in patients with postoperative early geological activity. About 2 years after surgery for follow-up in the discovery of daily life in patients with significant improvements, quality of life can be improved. Conclude Dynesys dynamic stabilization system is effective method of treatment of lumbar degenerative disc disease can have clinical efficacy and can effectively preserve fixed to the satisfaction of certain activity of lumbar segmental, prevents the degeneration of adjacent segments. |
